Generally, when I have clients that are morbidly obese or in extremely bad shape I start with almost a cardiac or occupational rehab mentality.
For instance, I may put a standard high back chair in the back of the gym and have them get up and sit down 10, 20, 50+ times per set depending on just how bad they really are.
This taxes their cardiovascular system and stimulates their cardio system, quads and hip flexors. One of my pet peeves in training clients of any ages are legs. Especially clients out of shape. You need good wheels as you get older!
This simple "get up" will really make them work hard.
You, then at some point, can add dumbells to each hand to make it more difficult and also start to stimulate the upper body to be involved.

I would definitely stress cardio more than anything and remind the client to stay physically active (i.e., taking stairs vice elevator, washing own car, cutting grass, vacuuming house, etc). Of course, watching what they eat, too.
